cp -r skills/skills/productivity/to-questionnaire ~/.claude/skills/to-questionnaire
Safety box score
Grade A, minimal risk: 4 of 100 risk points, rated 2026-10-16. The grade rates what the skill and its scripts can reach on the machine of whoever installs it, across eight categories from shell execution to secrets access. How the score works.
This skill only turns a user’s conversation into a questionnaire markdown file and writes it in the current directory. It does not ship any executable script or networked component; the main exposure is a small local file write plus whatever sensitive details the user chooses to include in the questionnaire text.
| Category | Level | Why |
|---|---|---|
| Code & shell execution | 0 (none) | No scripts, shell commands, or code evaluation are present; the skill is just a Markdown workflow described in `SKILL.md`. |
| Network access | 0 (none) | The files contain no download, upload, API, or remote-host behavior. |
| Filesystem reach | 1 (low) | It explicitly writes `to-questionnaire-<slug>.md` in the current directory, which is a bounded local file creation. |
| Secrets & credentials | 0 (none) | It does not request or access tokens, keys, passwords, `.env` files, or other credential stores. |
| Privilege & persistence | 0 (none) | There is no sudo, global install, service registration, or OS/shell configuration change. |
| Instruction hijack surface | 0 (none) | The prompt content is straightforward and does not tell the agent to ignore rules, hide things, or follow remote instructions. |
| Irreversible actions | 0 (none) | It only drafts a questionnaire for the user to hand over; it does not send, publish, delete, pay, or otherwise act on its own. |
| Transparency | 0 (none) | Everything provided is readable text; there are no binaries, encoded payloads, or unreviewable installers. |
Before installing
- Open `SKILL.md` and confirm the only side effect you want is a Markdown file in the current directory.
- Test once in a scratch folder to see exactly how the slug and filename are formed.
- Check whether the questionnaire text may include sensitive business or personal details before handing it to someone else.
